As per my understanding for reaching to the root cause of any problem ask only three "why?" questions. For example if your son got late for the school... ask him why he got late for the school? His answer might be because he got up late in morning.... then ask third why as why he get up late this morning? His answer might be because he was playing video game till late night.. so here is root cause. So he needs to sleep on time...
